I never been to many KTV only SANDS. SANDS mainly PRC girls and local girls. tip for PRC is usually about 1m RP to 1.5m RP. Very pretty girls but not many can take out based on what I have seen. The girls will sit with you throughout.. You can have dinner at the KTV, play pool then sing/gamble etc whole night till about 2am, later on weekends.

prc karaoke you can find in malio, suncity, alexis, illegals, etc
usually they set a voucher price for the lc, local around 500k prc around 1000k for 1 voucher.
Each location will have slightly different regulation and price so ask the mamasan if you are planing to do the girl.
outside the voucher price you usually tips the girl around 300-500k

Was in Jkt last week and look see Classic Hotel. It is as described - step in and the mamasan will try to hook u up with one of the girls.
