Please help me out - Virgin trip to Thailand for specials

I'm heading to Thailand for a short trip in September. Gf allows me to go for "special" massages. But I have never go for one before, so I need some expert help.

1. Which city got the best touristy spots AND massage parlours? I'm torn between Bangkok, Chiangmai, Pattaya and Phuket at the moment. I need more info on the latter.

2. I am keen to try out soapy massage and penis massage, how much should I be paying for each? I support $40-$100 is about the range?

3. What is the procedure like for such services? Is it ok if my gf enter the premises? I mean when I do my specials, can she do her legit massages?

Looking forward to really enjoy myself but not offend my gf by going to those fish tanks.
